Performance Review Template for Agencies and Studios
Most performance reviews fail because they surface surprises. A team member who receives critical feedback they did not expect during a quarterly review feels blindsided, not developed. Effective performance reviews document what has already been discussed throughout the period, not new information.
This template is structured to prevent the surprise problem: it requires goal tracking and ongoing documentation between review periods, so the review conversation covers familiar ground.
Quarterly Check-In Structure
The template uses quarterly reviews, not annual ones. Annual reviews have too much lag. Work that happened in February is poorly remembered in December. Quarterly reviews maintain a 90-day reference window where both manager and team member can recall specific projects, decisions, and outcomes.
Each quarterly review covers four sections: goal progress against the previous quarter's targets, output quality review with specific project examples, development areas with concrete next steps, and next-quarter goal setting. The last section feeds directly into the following review, creating a continuous cycle.
Goal Tracking Between Reviews
The template includes a goal-tracking task that runs throughout the quarter, not just at review time. Goals set in the review are added to a shared tracking document with owner, timeline, and mid-quarter check-in dates. Both manager and team member have access. This prevents the common failure where 90-day goals are set, filed, and forgotten until the next review.
Self-Assessment Format
The template includes a self-assessment section completed by the team member before the review conversation. Self-assessment serves two functions: it gives the team member ownership over the review rather than making it something that happens to them, and it surfaces gaps between self-perception and the manager's assessment that would otherwise emerge as conflict during the meeting.
